Dr. Nicole Kelleher is a dual board-certified physician in Physical Medicine & Rehabilitation and Obesity Medicine, with active medical licenses in California and Virginia. She currently serves as a Staff Physician at Dignity Health Northridge Medical Center (since January 2023), where she developed a new outpatient clinic and post-acute continuity program for patients recovering from stroke, traumatic brain injury (TBI), spinal cord injury (SCI), and complex neurological conditions.

Dr. Kelleher earned her MD and MPH from the University of Virginia (UVA), completing both degrees in 2013, with a public health concentration in Organizational Management and earning Distinguished Honors. She completed her residency in PM&R at UVA from 2013 to 2017, receiving multiple honors including Resident of the Year and achieving the highest in-service exam scores for three consecutive years. She went on to complete a subspecialty externship in Cancer Rehabilitation at Georgetown University and National Rehabilitation Hospital in 2017. In 2023, she became board-certified in Obesity Medicine.

From 2017 to 2020, Dr. Kelleher served as an Assistant Professor at UVA School of Medicine, where she developed the institution’s first outpatient oncology rehabilitation program. She also served as an attending physician in UVA’s 50-bed acute inpatient rehab hospital and led the inpatient consult service for neurorehabilitation and complex care coordination.
Dr. Kelleher was Chief Medical Officer at Lightyear Health (2020–2023), where she oversaw clinical operations, quality initiatives, and strategic provider growth across multiple states.

An accomplished educator and speaker, she has delivered over 20 academic lectures and presented clinical research nationally. Her background also includes service as a professional triathlete and elite triathlon coach, with multiple national and international podium finishes.

Dr. Kelleher continues to integrate evidence-based care with innovative, patient-focused rehabilitation strategies aimed at restoring function, enhancing quality of life, and promoting long-term wellness.
