Introduction

Dr. Thomas M. Phan is a board-certified interventional pain management physician and physiatrist with extensive training in advanced spine, nerve, and musculoskeletal procedures. He specializes in fluoroscopic and ultrasound-guided interventions, neuromodulation, minimally invasive spine procedures, and comprehensive rehabilitation-based pain management. Dr. Phan currently practices at Premier Pain Consultants in Fullerton and Irvine, California, where he has served as an Interventional Pain Management Physician since August 2025.

Dr. Phan earned his Doctor of Medicine from the University of Maryland School of Medicine in Baltimore (2011–2015). He completed his internship in Internal Medicine at St. Mary Medical Center in Long Beach, California (2015–2016), followed by his residency in Physical Medicine & Rehabilitation at Virginia Commonwealth University’s Medical College of Virginia (2016–2019). He then pursued advanced fellowship training in Pain Medicine at the same institution (2019–2020). He graduated summa cum laude from San Diego State University with a Bachelor of Science in Biology and Economics (2004–2009).

Board-certified in both Pain Medicine and Physical Medicine & Rehabilitation by the American Board of Physical Medicine & Rehabilitation since 2021, Dr. Phan holds active licensure in California, including a California Fluoroscopy Permit (issued 2025) and DEA registration. His previous medical licensure includes Maryland and Virginia.

Dr. Phan has extensive clinical experience, previously serving as an Interventional Pain Management Physician with The Centers for Advanced Orthopaedics in Maryland (2020–2025) and as a Pain Management and Post-Acute Rehabilitation Consultant at Meadow Park Rehabilitation & Healthcare in Catonsville, Maryland (2022–2025). He holds current hospital privileges at Providence St. Jude Medical Center in Fullerton (2025–present) and UCI Health Placentia Linda (2025–present) and has held prior appointments at Anne Arundel Medical Center and Baltimore Washington Medical Center.

An active contributor to the field, Dr. Phan has published peer-reviewed research on peripheral nerve stimulation and presented at national pain and rehabilitation conferences, including NANS, AAP, AAPM&R, and the World Congress on Regional Anesthesia and Pain Medicine. He is a member of AAPM&R, ASPN, ASRA, IPSIS, and the Interventional Orthobiologics Foundation.

Dr. Phan is fluent in English, conversational in Vietnamese, and has basic proficiency in Spanish.
