Q: Have any of your accreditations ever been investigated, suspended or removed? (if yes, explain)
A: Yes. On August 7th, 2007 I entered into a stipulated agreement with the Department of Professional Licensing (DOPL) of Utah for a term of two years with the potential to petition the board for a reduction to one year in regard to a consensual intimate relationship that occurred with a female patient. At the moment of realization of my lapse in judgment, I immediately self reported to my Division Chief and the University legal counsel. Because of the stipulated order from the State of Utah, my practice at the University of Utah School of Medicine and Primary Children’s Hospital required a chaperone and regular reports regarding compliance with stipulated restriction. I also resigned my position at the VA Salt Lake City Medical Center. Additionally, as a result of the stipulated order from the State of Utah, the states of New York and Illinois ordered concurrent probationary periods for respective medical licenses in each state. The concomitant and coterminous action taken by the state of New York prompted the New York Medicaid insurance to put me on a probationary period.
Once the stipulation and order was established, I met or exceeded all expectations of DOPL, my University, and myself. On September 24th, 2008, the State of Utah DOPL granted termination of the stipulated probationary period as a result of satisfying all the terms and conditions set forth on August 8th, 2007 Stipulation and Order. After, my license to practice medicine and surgery was reinstated without any limitations, I was reinstated with full privileges at all medical centers and all insurance carriers with which I had work, and I became the Chief of Otolaryngology at the George E. Wahlen Veterans Administration hospital for 4+ years. At this time, I practice medicine and surgery without restrictions on my license, and the only residual restriction is that I have not been able to be credentialed with Blue Shield of California for this event that happened over ten years prior.
